Instructions for Completing Census: (If you receive our Weekend Updates through email, you will be receiving an email with the census attached. You may find that the easiest way to complete and return.) Census opens best using Adobe.

OPTION 1 – To complete and return as an attachment:
1. Click on link.
2. Download and save form (make note of where you are saving it)
3. Close Browser tab
4. Go to where you saved the file and open it
5. Fill out the form. Type directly to the form.
6. After the form is completed, go to FILE>SAVE AS. Close.
7. Email your completed form as an attachment to office@saintpolycarp.org
OPTION 2 – To complete, print and return to office:
1. Click on link
2. Fill out the form. You can type directly into the form.
3. Print
4. Return printed census to office.
